Fellowship Place, a nonprofit agency located in New Haven, CT, is seeking a full time Case Manager for its Permanent Supportive Housing program. The Permanent Supportive Housing program provides housing and support services for individuals living with a mental illness and/or co-occurring substance use disorder. A majority of residents were formally homeless or living in transitional housing programs prior to moving to Fellowship Place. Duties include outreach, service coordination with primary care and behavioral health service providers, assistance with applications for government entitlement programs, referrals to employment and education programs, supportive counseling, service planning, and case notes. Successful candidates will have good motivational interviewing skills, knowledge of government entitlement programs and mental health services in greater New haven area, and basic understanding of harm reduction and housing first service models.
BA in Social Work or Psychology required; prior casework experience working with individuals with mental health disorders preferred. Must have valid driver’s license and use of personal car during work hours.
Hours: 9am-4:30pm, Monday-Friday with occasional overtime. Sal. Is $19.50/hour with paid time off, medical benefits, and retirement plan options.
