Full Time Residential Case Manager

Fellowship Place, a community based mental health agency, is recruiting for a Case Manager to provide a broad range of services for single adults who are living with a serious mental illness or co-occurring substance use disorder and mental illness and who reside in permanent supportive housing.  A majority of clients are formerly homeless individuals.  Responsibilities include intake, counseling, service planning, information & referral, life skills coaching, some transportation and escort service to medical appointments, and assistance with applying for & maintaining entitlements.

Bachelors degree in Social Work or related field.  Prior work experience in mental health and supportive housing preferred. Spanish speaking a plus.  Basic knowledge of Harm Reduction and Motivational Interviewing helpful. Must be a team player with good oral and written communication skills.  Driver’s license and use of car during work hours required.  Salary is $23 per hour with health benefits, paid time off, and 403-B Retirement Plan.
